Deduction Authorization- Working Dues
I hereby assign to the New York City and Vicinity District Council of the United Brotherhood of Carpenters and Joiners of America (the "District Council") and direct any employer by whom I am employed under a collectively bargained agreement with the District Council to deduct from my hourly wages the working dues as designated by the District Council's Bylaws and to forward weekly the deducted amounts to the District Council. This dues check-off authorization is voluntarily made. This voluntary dues check-off authorization is not conditioned upon my present or future membership in the District Council or lack of such membership. I understand that the signing of this dues check-off is not a condition of employment with an employer and that I have the right instead to pay my working dues directly to the District Council. This dues authorization shall remain in effect for one (1) year from the date of my signature and from year to year thereafter unless and until I withdraw it in writing upon ten (10) days notice prior to the end of each annual period, with such written notice given to the District Council and to my then current employer.
Deduction Authorization- Political Action Committee
I hereby assign to the New York City and Vicinity District Council of the United Brotherhood of Carpenters and Joiners of America Political Action Committee ("PAC") and authorize and direct each employer signatory to any collectively bargained agreement with the District Council to deduct from my paycheck Five Cents (0.05) per hour worked and remit that amount to the PAC. I understand that the money collected by the PAC will be used for PAC expenditures and distributions. This PAC authorization is voluntarily made. I understand that the signing of this authorization and the making of payments to the PAC are not conditions of membership in the District Council or of employment with an employer and that I have a right to refuse to sign this authorization for the PAC and to contribute to the PAC without reprisal and that the PAC may distribute the money it receives to any legally constituted political action committee and to make political expenditures and contributions in connection with State and Local elections. I also understand that this amount of money contributed to the PAC is merely a suggested guideline, that I am free to contribute more or less than this amount by any lawful means other than this check-off, and that the District Council cannot favor or disadvantage me because of the amount of my contribution or my decision not to contribute. My signature on this card shall be considered a revocation of any prior authorization for PAC deductions. I hereby affirm that I am not a foreign national. This PAC authorization shall remain in effect until revoked by me in writing.
